Welcome to our new practice member pages. You will find information here about what you can expect from your first visit to our local Winnipeg office.
Please print and complete the appropriate New Patient Health History
before coming in for your first appointment. If you are unable to print the below PDF, we will provide you with the necessary forms at your first visit.
New Patient Forms for your visit with Drs. Richard and Lyndsay Ledda:
- Adult Health History
- Pediatric Health History
- Pregnancy Health History
- Postpartum Health History
- MPI Health History
- WCB Injury Form (fill this form out in addition to the Health History form)
- Re-assement Form (this form is for returning patients who have not been to the office in 6 months or more)

If you have not already booked your appointment, call our office at 204-254-1313 and schedule your first visit. Please bring your completed forms with you. If you are unable to complete them prior to visiting our office, please arrive 15 minutes prior to your scheduled appointment.
